Sweepstakes is open to anyone 18 years or older at the time of entry. Employees of Cincy Business, its parent and their immediate families (spouse, parents, children, siblings and their spouses) and individuals living in the same household as such employees are ineligible. HOW TO ENTER: Sweepstakes begins on September 20, 2005, and ends on December 1, 2005 (“Sweepstakes Period”). During the Sweepstakes Period, fax this form to (866) 786-6318 or go to www.cincybusinessmag.com. Limit one entry during Sweepstakes Period.

HOW TO WIN: At the end of the Sweepstakes Period, one name will be drawn at random from all eligible entries received.

PRIZES AND ODDS: There will be one winner. The winner will receive 2 round-trip airline tickets and hotel stay at the Westgate Lakes Resort in Orlando, Florida valued at approximately $900.00. Reservations can only be made 30 days or less prior to departure and are subject to availability. (Excluding holidays and blackout periods). Certificate is non-transferable. TRAVEL MUST BE COMPLETED BY July 1, 2006. Once reservations are confirmed, no changes will be permitted. The odds of winning are approximately one in 400.

NOTIFICATION: Winner will be notified by telephone and will be required to accept prize within 5 days of notification. Failure to accept within 5 days will result in forfeiture of prize.
